Team Bank Account Authorization Letters
- Any team opening a team bank account will require 3 signing officers (none from the same family).
- Contact the bank of your choice to make an appointment to open a team bank account
- Complete the authorization letter (manager or coach to complete)- see the attached info and authorization letter
- Email completed form for authorization to info@kmha.ca When authorized it will be emailed back to you, Email or take in person the authorization letter to the bank
- All team accounts must be closed at the end of the season and not carried forward.

Special Event Permit
The purpose of a sanctioning a special event is to extend Hockey Canada insurance program coverage such as major medical/dental coverage to activities that do not fall under regularly scheduled hockey programming. Special event sanctions are for usage of events such as dryland training, fundraisers, team parties, team bonding events, extra ice you have purchased for your team and other activities outside of regular hockey programming. Not all activities are eligible for coverage.
